The Root Cause Coalition Welcomes Center for Population Health at Cheshire Medical Center as Newest Member

WASHINGTON, DC – The Root Cause Coalition welcomed the Center for Population Health (CPH) at Cheshire Medical Center as its newest member, bringing the Coalition’s total membership to 51 organizations.

Located in Keene, New Hampshire, Cheshire Medical Center is a nonprofit community medical center and member of the Dartmouth-Hitchcock Health system. Cheshire Medical Center provides a continuum of care spanning primary care and specialty medicine to surgical services and acute inpatient care. CPH is a department of Cheshire Medical Center focused on health and wellness education, outreach and support to individuals, families, worksites, schools and the community. Through CPH, Cheshire Medical Center works toward eliminating the barriers to good health, developing new opportunities for collaboration with local partners and resources, and supporting the organizational needs of the local Council for a Healthier Community and the Greater Monadnock Public Health Network, both focused on population health.

The CPH at Cheshire Medical Center became the eighth organization to join the Coalition in 2018, following the University of Pittsburgh Medical Center, Second Harvest Heartland, LISC, the Children’s Hospital of Michigan Foundation, the Alliance for Strong Families and Communities, Fairview Health Services and Aetna.
